What we collect

The intake form asks for your name, contact details, a preferred date, the formula you are curious about, your goals, and anything you want your nurse to know. We collect nothing you do not type.

How we use it

• To have a nurse review your intake before your visit.
• To confirm your appointment by email or phone.
• To keep a clinical record of your sessions.

Who sees it

The nurse assigned to your session and our clinical oversight lead. We do not sell or share your information with advertisers.

Your choices
